Well Connection
Learn about Well Connection, BCBSMA’s Telemedicine provider. Telemedicine allows you, or your dependents to see a doctor, licensed in the state you are calling from, virtually via phone or video call. If you have the Gold plan, you will pay the office visit copay listed on your Summary of Benefits and Coverage to use Well Connection. Visits to a telemedicine provider are typically less than seeing your primary care physician or going to the emergency room or urgent care. This will accumulate towards your out-of-pocket maximum. It is highly recommended you register for Well Connection before needing the service. It will save you time when you really need the service.

My Health Reimbursement Arrangement Benefits
A Health Reimbursement Arrangement (HRA) is a promise to reimburse or pay a specific amount toward insurable medical expenses incurred by a participant
HP Cummings Construction Co. will contribute $1,200 for singles or $2,400 for family to cover the cost of deductible expenses in the form of a cost share.
HRC Total Solutions will continue to administer the HRA. Employees will be paid directly from your HRA by HRC Total Solutions when funds are available.
If you enter the plan at a time other than the beginning of the plan year, the amount credited to your account will be reduced to reflect the time of actual participation.
My Group Life and AD&D Benefits
HP Cummings gives eligible employees Life and Accidental Death & Dismemberment coverage in the amount of $25,000 or $50,000 based on level of coverage based on employee class.
Contributions
This is 100% Employer paid.

Supplemental Life and AD&D Benefits
Short Term Disability Benefits
HP Cummings offers Voluntary Short Term Disability coverage. The benefits pay out if you are disabled on the 15th day due to an accident or illness. The benefits pay up to 66 2/3% of your weekly pre-disability income, up to a limit of $750 per week. The maximum benefits period is 11 weeks.

My Long Term Disability Benefits
HP Cummings wants to help you maintain an income when you need it the most. They offer employees a long term disability plan that will cover 60% of your salary to a maximum of $10,000 or $6,000 based on your employee class.

My Voluntary Benefits - Aflac
HP Cummings offers employees the ability to enroll in Voluntary benefits.
Everyone’s benefit needs are different. That’s why it’s important to choose the benefits that are right for your personal situation.
- Contract is between Aflac and the employee, HPC allows for payroll deductions for payment of insurance coverage.
- Coverage is available for your spouse and eligible dependents.
- Benefits are paid directly to you, unless you specify otherwise.
- You may receive benefits regardless of any other insurance you may have with other insurance companies.
Dental Insurance — Benefits for dental care.
Accident Insurance — Helps offset unexpected medical expenses, such as emergency room fees, deductibles and co-payments that can result from a fracture, dislocation or other covered accidental injury.
Aflac Plus Rider – Pays a lump sum along with additional benefits when you are diagnosed with a covered health event.
Cancer Insurance — Helps offset the out-of-pocket medical and indirect, non-medical expenses related to cancer that most medical plans don’t cover.
Vision – Benefits for eye care
Hospital – Helps pay for the confinement to hospital for any sickness, injury or maternity.

My Retirement Benefits
EE Deferral: Auto enrollment provision of 4% with automatic 1% increases up to 10%. Can be changed by employee.
ER Match: 50% up to 3% of employee’s gross wages for plan participants. 100% vested on contribution.
Profit Sharing disbursements are deposited directly into your 401(k) and are applicable to the vesting schedule.
Contributions
Between 1% and 100% of salary up to the Internal Revenue Service limits. For 2019 the annual contribution is $19,500. If you reach the age of 50 anytime during the calendar year or are over 50 you may make additional “catch-up” contributions. For 2020, you can make “catch-up” contributions of up to $6,500. 2021 limits are anticipated to remain the same as 2020.
Eligibility
All full and part time employees are eligible to contribute to the 401(k) &/or Roth IRA plan on the 1st of the month following 90 day eligibility.
For Profit Sharing:
Employees are eligible on the closer of January 1st or July 1st after 1 full year of employment.
For Example: Hire Date of March 15, 2018 would be eligible at July 1, 2019.
Hire Date of 7/15/2018 would be eligible at January 1, 2020.
